Registered Nurse Assessor – UAS Assessments (NYIAP Program)

Job Overview:

Are you a Registered Nurse looking to expand your skills and gain expertise in UAS assessments? We are hiring Registered Nurse Assessors for the New York Independent Assessor Program (NYIAP). This is an excellent opportunity for nurses who want to learn how to conduct Uniform Assessment System (UAS) evaluations and transition into a role that combines clinical expertise with assessment-based care planning.

In this position, you’ll receive comprehensive training to conduct health assessments for Medicaid recipients (ages 19+), helping determine their eligibility for long-term community-based services. Assessments are performed via telehealth and in-person home visits, with a flexible schedule based on consumer-driven appointments.

Key Responsibilities:

• Receive training and mentorship to conduct telehealth and in-home UAS assessments.
• Review and summarize medical records to determine care needs and eligibility.
• Learn how to document assessments in the UAS-NY online system.
• Collaborate with the clinical team to ensure accurate evaluations.
• Stay informed on state guidelines and assessment regulations.
• Travel within your assigned region for in-home visits.

Requirements:

• Active RN license in New York State.
• At least 2 years of 1 to 1 Patient Care in the Patients Home
• Strong organizational and communication skills.
• Basic proficiency in Microsoft Word, Outlook, and Zoom.
• Valid New York State ID and ability to travel for in-home visits.
• Reliable high-speed internet for telehealth assessments.
